Assessment Checklist (V)
The Assessment Checklist (Victim) must be completed to allow you to move the case forward to Ongoing Support status.
First, select the case type from the first dropdown and choose 'Victim'. The system will present you with three further questions that require answers.
Demographics
Nineteen specific demographics are a standard requirement; however, we also want the client profile to be completed as fully as possible. This provides us with the information we need to ensure that we are providing a service to all areas of the community and operating inclusively. The required demographics are all marked with an asterisk (*) in the client profile.
If you are unable to complete the demographics for any reason, choose from the two options with the most accurate response between Most Fields Complete or Required Fields Incomplete. You will be asked to provide a reason why they are incomplete from a dropdown list.
Safe Lives Assessment
To progress to Ongoing Support, you need to have completed a Safe Lives Assessment with the client. We would be unable to provide comprehensive support if this has not been fully completed.
Individual Safety and Support Planning
This may take several forms, depending on the level of contact you have with the client; however, it is essential in all cases that some level of Safety and Support planning occurs. This should be added to the system, either with notes or by attaching a copy of any document sent to the client.
If you need to complete more work before finishing the checklist, you can save it and return to it later, just like other assessments, by clicking the SAVE button. This will allow you to resume the checklist at a later time.
Once all the above answers have been supplied, you can complete the assessment. An option will then appear to let you move the case forward to Ongoing Support.
