There are several required fields on the system. Most of these fields are not enforced by the system. However, there is still a requirement for our workers to collect this information to ensure that our services can demonstrate they are operating in a manner that makes them inclusive to the entire community we serve.

Client Profile Required Fields

The following list contains all the mandatory fields in the client profile for all clients.

05. Last Name

06. Date of Birth

07. Ethnicity

08. Nationality

09. Religion

10. Gender At Birth

11. Gender Now

12. Sexual Orientation

13. Housing Status

14. Employment Status

15. Immigration Status

16. In Receipt of Benefits

17. Parental Status

  • 18. Number of Children Living with You
  • 19. Number of Children Living Elsewhere

The above two questions (18. & 19.) only appear if you select 'Parent' in the  Parental Status question.

20. Child(ren) on Care Plan, or in Care
