Closure Checklist

A closure checklist must be completed before a case can be closed. This checklist is designed to ensure that we have completed all the tasks required by the system before closing the case.

The steps to be completed are:

Choose Case Type (All Cases)

We are unable to identify the case type at this point, so you need to choose between Perpetrator/Victim/CYP or Other which will then give you the correct checklist to complete.

2. Demographics Checked? (All Cases)

Nineteen specific demographics are a standard requirement; however, we also want the client profile to be completed as fully as possible. This provides us with the information we need to ensure that we are providing a service to all areas of the community and operating inclusively. The required demographics are all marked with an asterisk (*) in the client profile. 

If you are unable to complete the demographics for any reason, choose from the two options with the most accurate response between Most Fields Complete or Required Fields Incomplete. You will be asked to provide a reason why they are incomplete from a dropdown list.

3. Update Referrer? (All Cases)

You should email the referrer to let them know that you are closing the case. Add the body of the email to the Update Referrer case action. Where the client may have dropped out or not engaged, it's suggested that you wait at least a week before closing the case, because the referrer may be able to make contact with the client and get them to engage or re-engage, so it's easier to reopen the case from Closure than it is from Closed.

4. Are all Assessments Completed?

In the case journal on the system, you will have a list of all case actions and assessments added to the system. Any assessment added to the system must be completed before the case is closed. This means that every entry in the journal should have a purple tick next to it and be marked complete.

If you have any entries with a Green Arrow, Amber Triangle or Red Exclamation Mark, these assessments need to be resolved and completed before you close the case. If you are unable to complete these assessments, please log a Helpdesk ticket to have them resolved, and then close the case.

5. Partner Support Worker Advised? (Perpetrator Cases Only)

Have you informed the Partner Support worker that the Perpetrator case is being closed? You should be able to find the link to the case or the Victim in the case details. You can then email the Partner Support Worker so that they can take the appropriate action with the victim and their case.

6. Final Evaluation Completed? (Perpetrator Cases Only)

All Perpetrators need to complete a Final Evaluation form before we close their case. The results from this need to be added to the system. 

If you need to complete more work before finishing the checklist, you can save it and return to it later, just like other assessments, by clicking the SAVE button. This will allow you to resume the checklist at a later time.

Once all the above answers have been supplied, you can complete the assessment. An option will then appear to let you close the case and select a Primary Outcome (Required).
